Documentation improvements

Since SSH_OPTIONS_STATUS_CALLBACK doesn't seem to be implemented
anymore, any documentation for it should be removed.

Also, the tutorial should be updated to remove references to
deprecated functions (e.g. ssh_is_server_known). The functions that
replace them in turn should be documented.
